Same old story, contractors cutting corners and then getting retrained. How many chances do they get?

As someone who works in rail, I can tell you it's not that simple. Sometimes retraining fixes the issue, sometimes not.
I wonder if the retraining included any actual hands-on practice or just a video and a pat on the back.
